Background Check Authorization and Consent

I hereby authorize Pleasant View Baptist Church – Covington, GA to conduct a background check as part of the employment and/or volunteer screening process. I understand that this background check may include, but is not limited to, criminal history and other information deemed relevant to determine my eligibility for employment or volunteer service.

I acknowledge and agree that the results of this background check will remain confidential and will only be reviewed by authorized personnel designated by Pleasant View Baptist Church. I understand that the information obtained will be used solely for evaluation purposes.

I further understand that the results of the background check may impact the decision regarding my ability to be employed and/or serve as a volunteer at Pleasant View Baptist Church – Covington, GA.
